This is impingement.
The problem is fundamentally biomechanical: the subacromial space is narrowed by correctable postural and neuromuscular factors. This is why physiotherapy — which corrects those factors directly — is not just equivalent to surgery for shoulder impingement but is the more logical first-line treatment. Surgery removes part of the acromion (acromioplasty) to create more space, but does nothing to improve the scapular control and rotator cuff activation that would keep the space open under load. Without physiotherapy after surgery, the functional impingement pattern persists in the surgically enlarged space.
